Documentation Steps Required for STAR Awards
1. Download the appropriate STAR Awards Report Form
2. Fill out all the required information.
3. Take the STAR form to your event or activity, and upon completion of the activity, have the show secretary or an event official sign it.
4. Mail the STAR form and any required documentation to the IPSHR Star Award Secretary.
